What are the cirstances for being medically discharged from the military for bipolar disorder?

I am in the military, I've been diagnosed recently with mixed episode bipolar disorder. I have a strong desire to be medically separated, but I'm afraid to ask for it because I'm afraid to be screwed over by the military...Being in the military has caused me too much stress, and being in it is causing me to not function as I once did, and I can't handle it anymore. I did originally tell them that I did not wish to be separated because even after multiple regiments with different medications, my mood is steadily worsting. I've done what I can and have given my psychologist informed (even kept a notebook on how each med effected me) and have talked about what's going on, but no improvements on my mental stability has been made. How should I approach this? What can be done to see that I'm medically separated?
